投標(biāo)人應(yīng)具備的資格或業(yè)績：①投標(biāo)人需提供有效的營業(yè)執(zhí)照、稅務(wù)登記證和組織機構(gòu)代碼證（或三證合一）； ②投標(biāo)人需提供投標(biāo)機型的醫(yī)療器械注冊證或備案憑證； ③投標(biāo)人應(yīng)提供醫(yī)療器械生產(chǎn)企業(yè)許可證（如為制造商投標(biāo)）； ④投標(biāo)人應(yīng)有制造商（或地區(qū)總代理）出具的授權(quán)書和醫(yī)療器械經(jīng)營企業(yè)許可證（如為代理（經(jīng)銷）商投標(biāo)）或備案許可證； 如投標(biāo)人為境外企業(yè)，須在投標(biāo)截止時間前與境內(nèi)具有進出口經(jīng)營權(quán)的貿(mào)易公司簽訂進出口協(xié)議，并將該協(xié)議放入投標(biāo)文件，另外需提供該貿(mào)易公司的上述資格證明文件。 ⑤投標(biāo)人如為代理（經(jīng)銷）商，則須出具貨物制造商針對本項目的授權(quán)書。 注：以上材料報名時須提供原件或有效復(fù)印件。

3.Qualification Requirements For Bidder
Qualifications or Performance：① The bidder shall provide valid business license, tax registration certificate and organization code certificate (or three certificates in one); ② The bidder shall provide the medical device registration certificate or filing certificate of the bidding model; ③ The bidder shall provide medical device manufacturer license (if bidding for manufacturer); ④ The bidder shall have the power of attorney issued by the manufacturer (or regional general agent) and the medical device business enterprise license (such as bidding for agent (distributor) or filing license); If the bidder is an overseas enterprise, it is required to sign an import and export agreement with a domestic trading company with import and export operation rights before the bidding deadline, and put the agreement into the bidding documents. In addition, the above qualification certificates of the trading company shall be provided. ⑤ If the bidder is an agent (distributor), he shall issue the power of attorney of the goods manufacturer for the project. Note: the original or valid copy of the above materials should be provided.
